Partial Thickness Rotator Cuff Tear-Avalanche Sign- Florida-Pearls-AANA
Dr.Bennett from Sarasota, Florida notes that orthopedic surgeons often use the width of the partial tear of the supraspinatus tendon to determine whether to fix a partial thickness tear of the rotator cuff and also to guide toward "complete" the tear or "in-situ" repair. He coined a simple term that may help make a decision, "the Avalanche Sign". With the arthroscope in the subacromial space and the neuroprobe penetrated through the leading edge of the supraspinatus tendon-marked with a prolene- the arm is placed at the side and forced adduction into the side puts the rotator cuff under tension. The neuroprobe then is forcefully moved front to back and back to front. If the tendon move with the probe, the underlying cross sectional fiber insertion of the tendon is incompetent and Dr Bennett advises tear completion. -
